1.Cub Scout Personal Information Form. This form enables the den leader to store information about each cub.

2. Cub Scout Application. This is the application that needs to be filled out when a boy want to join the cub scouts.

3.Local Tour Permit. This is a permission form allowing your child to go on local outings. Your den leader will let you know if you need this one.

4. Medical Form Class 1 & 2. This is a medical history form for a cub scout.

5. Medical Form Class 3. This is a second medical history form for a cub scout.
